A. Parkinsonian tremor, muscular rigidity, or akinesia developing in association with medication use (most commonly neuroleptic/antipsychotic or dopamine-depleting agent) B. Symptoms developed within a few weeks of starting or raising dose of causative medication
A. Dopamine antagonist exposure (antipsychotic or antiemetic) and development of severe muscle rigidity and elevated temperature B. ≥2 of: diaphoresis; dysphagia; tremor; incontinence; change in level of consciousness (confusion to coma); mutism; tachycardia; elevated/labile BP; leukocytosis; elevated CK C. Not attributable to another substance or medical/neurological condition
With predominant rigidity/bradykinesia; With prominent tremor; Associated with antipsychotic exposure; Associated with other dopamine-interfering medication
Associated with antipsychotic exposure; Associated with dopaminergic withdrawal; With severe rhabdomyolysis; Severity (Mild / Moderate / Severe)
Idiopathic Parkinson's disease (insidious onset; no medication exposure; persists after cessation; DaTscan abnormal); Lewy body NCD (dementia + parkinsonism + visual hallucinations); Drug-induced vs unmasked PD (does not resolve weeks after cessation - consider underlying PD)
Serotonin syndrome (hyperthermia + clonus + diaphoresis + hyperreflexia - starts within 24 hrs; clonus distinguishes from NMS rigidity); Malignant hyperthermia (anaesthetic trigger; volatile agents/suxamethonium; ryanodine receptor mutation); Lethal catatonia (treatment with ECT; NMS and lethal catatonia may overlap)
