Depressed mood most of the day, more days than not, for ≥2 years (≥1 yr children/adolescents)
≥5 of 9 SIG-E-CAPS symptoms for ≥2 weeks; ≥1 must be depressed mood or anhedonia
B. ≥2 of: poor appetite/overeating; insomnia/hypersomnia; low energy; low self-esteem; poor concentration; hopelessness C. No symptom-free period >2 months in the 2-year span D. MDE criteria may be continuously met throughout the 2-year period (double depression)
B. Clinically significant distress or impairment in functioning C. Not attributable to substances or another medical condition D. Not better explained by schizoaffective/schizophrenia spectrum disorder

With pure dysthymic syndrome; With persistent major depressive episode; With intermittent MDEs, with current episode; With intermittent MDEs, without current episode
Single episode / Recurrent; Mild / Moderate / Severe; With psychotic features; In partial remission / In full remission
MDD (full episode criteria met - not chronic subthreshold); Bipolar II with persistent depression; Cyclothymic disorder
Bipolar I or II disorder (prior manic or hypomanic episode - always ask before treating); Persistent depressive disorder / dysthymia (milder, chronic > 2 years); Grief/bereavement (proportionate; preserved pleasure)
