When this fits

There are episodes of detachment, derealisation, depersonalisation, time loss, or altered awareness.
Disconnection appears when arousal, trauma cues, shame, or threat exceed coping capacity.
The person may look present but later reports gaps, unreality, or watching from outside themselves.

When this may not fit

The person is deliberately avoiding while awareness and memory remain intact.
Symptoms are better explained by seizure, intoxication, delirium, sleep disorder, or psychosis.
The main issue is emotional numbing without altered awareness or integration.
Alternative explanations to exclude: seizure, syncope, delirium, intoxication, sleep phenomena, cognitive disorder, psychosis and behavioural disengagement. A trauma history does not remove these alternatives.

Pace trauma processing carefully
Attend to immediate safety, orientation and pacing. The PTSD meta-analysis found no overall association between pretreatment dissociation and psychotherapy effectiveness, so dissociation alone does not justify automatic prolonged postponement of trauma-focused treatment. This is not proof that every severe dissociative presentation or acutely unsafe patient is ready for immediate trauma processing. Consult the Australian PTSD guideline pathway for the actual treatment decision.
